摘要: AIMTodescribemagneticresonance(MR)imagingfeaturesofpancreaticneuroendocrineneoplasms(PanNENs)accordingtotheirgradeandtumor-nodes-metastasesstagebycomparingthemtohistopathologyandtodeterminetheaccuracyofMRimagingfeaturesinpredictingtheirbiologicalbehavior.METHODSThisstudywasapprovedbyourinstitutionalreviewboard;requirementforinformedpatientconsentwaswaivedduetotheretrospectivenatureofthestudy.PreoperativeMRexaminationsof55PanNENpatients(29men,26women;meanageof57.6years,range21-83years)performedbetweenJune2013andDecember2015werereviewed.Qualitativeandquantitativefeatureswerecomparedbetweentumorgradesandstagesdeterminedbyhistopathologicalanalysis.RESULTSIlldefinedmarginsweremorecommoninG2-3andstageⅢ-ⅣPanNENsthaninG1andlow-stagetumors(P<0.001);thisfeaturehadhighspecificityintheidentificationofG2-3andstageⅢ-Ⅳtumors(90.3%and96%,95%CI:73.1-97.5and77.7-99.8).ThemeanapparentdiffusioncoefficientvaluewassignificantlylowerinG2-3andstageⅢ-Ⅳlesionscomparedtowelldifferentiatedandlow-stagetumors(1.09×10-3mm2/svs1.45×10-3mm2/sand1.10×10-3mm2/svs1.53×10-3mm2/s,P=0.003and0.001).Receivingoperatorcharacteristicanalysisdeterminedoptimalcutoffsof1.21and1.28×10-3mm2/sfortheidentificationofG2-3andstageⅢ-Ⅳtumors,withsensitivityandspecificityvaluesof70.8/80.7%and64.5/64%(95%CI:48.7-86.6/60-92.7and45.4-80.2/42.6-81.3).CONCLUSIONMRfeaturesofPanNENsvaryaccordingtotheirgradeofdifferentiationandtheirstageatdiagnosisandcouldpredictthebiologicalbehaviorofthesetumors. ...
