摘要： Theemergenceofdaptomycinnon-susceptibleenterococci(DNSE)posesbothtreatmentandinfectioncontrolchallenges.CliniciansshouldbevigilantthatDNSEmaybeisolatedfrompatientswithorwithout(denovoDNSE)prioruseofdaptomycin.RecentepidemiologicaldatasuggestthepresenceofacommunityreservoirforDNSEwhichmaybeassociatedwithenvironmental,foodborneandagriculturalexposures.ThemechanismsofnonsusceptibilitytodaptomycinhavenotbeenwellcharacterizedandmaynotparallelthoseforStaphylococcusaureus.Theidentificationofdaptomycinresistancegenesinanaerobes,infarmanimalsandinanecosystemthathasbeenisolatedformillionyears,suggestthattheenvironmentalreservoirfordenovoDNSEmaybelargerthanpreviouslythought.Herein,thelimitedavailablescientificevidenceregardingthepossibleoriginofdenovoDNSEisdiscussed.ThecurrentexistingevidenceisnotsufficienttodrawfirmconclusionsontheoriginofDNSE.Furtherstudiestodeterminethemechanismsofdenovodaptomycinnonsusceptibilityamongenterococciareneeded. ... （共7頁）
